2. Complete TTC Ph2 Evaluation Form by Part-1 Teacher (with 3 years experience as Part-1 Teacher)
3. Medical Certificate by allopathic or ayurvedic doctor
4. Seva / Acitivity Report duly signed by the Recommending Teacher
5. Full Length Photo

For 10 Days YES+ TTC Program (For Part1 Teachers as mentioned above)
1. Complete TTC Ph2 Application
2. Complete TTC Ph2 Evaluation Form by Part-1 Teacher (with 3 years experience as Part-1 Teacher)
3. Medical Certificate by allopathic or ayurvedic doctor
4. Seva / Acitivity Report duly signed by the Recommending Teacher

The last date of submission of applications with evaluations (through all State Apex Offices / Country Co-ordinators) at TTC Desk Office, Bangalore Ashram : 6th Oct 2010 Wednesday
